Crusted scabies is the most severe presentation of infection related to the bite of the mite Sarcoptes scabiei var hominis. It is usually seen in patients with an underlying immunosuppressive condition. Case reports and studies suggest that infection with human T-cell lymphotropic virus 1 (HTLV-1) is a major predisposing factor to S scabiei infestation. This case report describes severe crusted scabies in a 17-year-old West Indian patient who was HTLV-1positive but HIV-negative. He responded well to treatment with sulfur and salicylic acid ointment and 5% permethrin cream but later died of complications from HTLV-1 infection. [Infect Med. 2009;26:126-128]
